Pitching To a Magazine

I've written an article about life with a chronic illness and I want to pitch it to a couple of UK magazines that have previously run features about health problems and other such things. But it will be the first time I've ever done it and I'm not sure whether there are certain conventions I should I send them a rough draft? Do I send pictures with a rough draft? Or do I just give an outline of the article? Do I mention pay? If I mention pay, do I tell them what I'd like to be paid for it? How am I to know what a reasonable figure would be?

Thanks ever so!
